CLR HEALTH AGENCY, LLC ("CLR HEALTH AGENCY", "we") operates this website. This policy describes what we collect here and how it is used.
The quote form
When you submit the quote form, we collect the name and phone number you enter, and — if you choose to provide them — your email address and a note about what you're looking for. The submission is delivered to us as a single email through our messaging platform's email delivery provider. If you check the marketing box, our messaging platform also keeps a record of that consent — the phone number you entered, this site's address, and the date and time — as our evidence that you gave it; nothing else from your submission is kept beyond that email. Our messaging platform also holds the sender's network (IP) address briefly to limit abuse of the form — no longer than about twenty minutes — and does not record it permanently.
We use what you submit to respond to your request and discuss your insurance options. We do not sell your information, and we do not share it for other companies' marketing.
Text Messaging & Mobile Opt-In
CLR HEALTH AGENCY operates two separate text message programs, and you opt in to each one independently by checking its own box on our quote form. Both boxes are unchecked by default, both are optional, neither is a condition of receiving a quote or purchasing anything, and checking one does not opt you into the other.
